Job summary

AccentCare Inc. is seeking a Field Supervisor for the Will and Cook Counties area. The role requires leadership of Home Care Aides, field visits, coaching, and ensuring care plans are followed during client visits.

Applicants should have a high school diploma or GED, 2 years of home care/CNA experience, reliable transportation, and a valid driver's license. Bilingual skills are a plus. This on-site role offers travel time responsibilities and mileage reimbursement.

Qualifications

• Strong communication, organizational, and problem-solving skills.
• Must have valid driver's license, reliable transportation, and auto insurance.
• Bilingual skills a plus, but not required.

Responsibilities

• Provides supervision and leadership to Home Care Aides
• Evaluate the quality, effectiveness and of services that are needed for each of the clients served by HCA
• Conduct unannounced quarterly home visits with clients that have Paid Family Caregivers to ensure the plan of care is being followed and HCA's are present during the hours scheduled.
• Coach, counsel, mentor and evaluate the performance of HCA's
• Conduct home visits as necessary, to provide hands on training and assistance
• Initiate and/or participate in clientstaffing discussion with Care Coordinator
• Investigate allegations of fraud and theft reported by HCA
• Documentation and follow up on incident reports
