Job summary

The Cleveland Clinic Main Campus in Independence, OH is seeking a Nursing Admission Specialist to initiate and resume home care for patients of all ages in the east region. You will perform comprehensive assessments, develop care plans, and provide ongoing support to promote patient independence and effective care management.

Qualifications

• Graduate of an accredited school of Professional Nursing.
• One year of current clinical experience as an RN.
• One year of experience in an acute medical/surgical or home care setting.
• Current state licensure as an RN.
• BLS certification through AHA or Red Cross.
• Valid driver's license on hire.
• Proof of automobile insurance.

Responsibilities

• Conduct and submit a complete and accurate Comprehensive Assessment per OASIS requirements.
• Identify immediate care and support needs and obtain orders for psychosocial and treatment priorities.
• Make admission decisions based on eligibility, acuity, and safety.
• Obtain consent for care.
• Demonstrate knowledge of coverage criteria based on payer source.
