Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) Certification through the American Heart Association (AHA).
• Valid Ohio Driver's License upon hire and proof of automobile insurance.

Responsibilities

• Provide direct nursing care to patients in their homes.
• Perform admission assessments and reassessments.
• Assess patients for safety risks, abuse, and neglect.
• Coordinate all disciplines for well-coordinated patient care.
• Manage patients’ pain and document care provided.
• Complete documentation and follow-up calls after visits.
• Attend meetings and coordinate patient care.
• Arrange visit schedules for patients.
