Loyola Medicine Home Care and Hospice, part of Trinity Health At Home, seeks a Speech Pathologist for in-home, one-to-one patient care in the Illinois service area. Qualifications

• CCC-SLP (ASHA) required or in supervised experience for licensure in IL.
• Eligibility for Illinois licensure or supervised experience in IL must be in progress if applicable.
• Meets educational and experience requirements for Speech-Language Pathology in home care.

Responsibilities

• Provide individualized speech therapy services in patients’ homes.
• Deliver patient-centered, compassionate care.
• Collaborate with interdisciplinary team to support care plans.
• Utilize advanced and easy-to-use clinical technology.
• Maintain accurate clinical documentation.
• Demonstrate flexibility, strong communication skills and accountability.
