Qualifications

• Meets the educational requirements for CCC-SLP or is in the process of obtaining licensure/registration in Illinois.
• Experience in home care and autonomous work environment preferred.

Responsibilities

• Provide one-to-one care with patients in their homes
• Collaborate with physicians, nursing teammates, social workers and home health aides
• Deliver compassionate, patient-centered speech pathology services
• Utilize technology and documentation tools to monitor progress
