Job summary

Loyola Medicine Home Care and Hospice, part of Trinity Health At Home, seeks a Speech Pathologist for in-home, one-to-one patient care in the Illinois service area. The role emphasizes collaboration with physicians, nurses and social workers, delivering high-quality care using advanced technology in a home setting.

Candidates must be motivated, detail-oriented, organized, tech‑savvy and able to work autonomously.

Qualifications

• CCC-SLP (ASHA) required or in supervised experience for licensure in IL.
• Eligibility for Illinois licensure or supervised experience in IL must be in progress if applicable.
• Meets educational and experience requirements for Speech-Language Pathology in home care.

Responsibilities

• Provide individualized speech therapy services in patients’ homes.
• Deliver patient-centered, compassionate care.
• Collaborate with interdisciplinary team to support care plans.
• Utilize advanced and easy-to-use clinical technology.
• Maintain accurate clinical documentation.

Position Summary

The Speech Pathologist provides one‑to‑one, patient‑centered care using advanced technology and clinical expertise in the home setting. Care is delivered through a multidisciplinary approach, collaborating with physicians, nursing staff, social workers and home health aides.

Candidates must be highly motivated, detail‑oriented, organized, technologically proficient and comfortable working autonomously.

Minimum Qualifications

• Meets educational and experience requirements for Certificate of Clinical Competence in Speech Pathology (CCC‑SLP) from ASHA
  OR
• Meets educational requirements and is currently completing the supervised experience required for certification, registration or licensure in the State of Illinois (if applicable)
