Job summary

Loyola Medicine Home Care and Hospice, part of Trinity Health At Home, is hiring a Speech Pathologist for contingent, PRN work delivering one-on-one therapy in patients’ homes, collaborating with physicians, nurses and social workers.

The role requires CCC-SLP eligibility or current supervised experience for licensure in Illinois; candidates must be motivated, organized, tech-savvy and able to work autonomously with compassionate, patient-centered care.

Qualifications

• Meets educational and experience requirements for CCC-SLP from ASHA.
• Or currently completing the supervised experience required for certification, registration or licensure in the State of Illinois (if applicable)

Responsibilities

• Provide individualized speech therapy services in patients’ homes
• Deliver patient-centered, compassionate care
• Collaborate with interdisciplinary team members to support comprehensive care plans
• Utilize advanced and easy-to-use clinical technology
• Maintain accurate clinical documentation
• Demonstrate flexibility, strong communication skills and professional accountability
