Job summary

Alternate Solutions Health Network is seeking a Physical Therapist Assistant I (PTA I) to deliver comprehensive skilled services under the direction of a Physical Therapist in patients' homes across Ohio. You will educate patients and families, ensure safety, and maintain compliant documentation as you support functional goals.

The PTA I collaborates with physicians and teammates, participates in case conferences, and adapts care plans to achieve maximum function while following state practice

Qualifications

• Minimum of one year experience as a PTA in an acute setting is required.
• Home care experience is desired.
• Able to communicate effectively with patients, families, physicians and coworkers.
• Knowledgeable on therapy best practices and clinical judgment.
• Detail-oriented, organized and timely in documentation and care.

Responsibilities

• Provide skilled PTA services in patients' homes under a Physical Therapist's plan of care.
• Observe, document and report patient progress and changes.
• Perform PT modalities and therapeutic procedures per physician order.
• Educate patient and family on therapies and safety.
• Ensure accurate documentation and compliance with Medicare/agency guidelines.
• Coordinate with the care team and participate in case conferences.
