Job summary

Alternate Solutions Health Network seeks a Physical Therapist Assistant I to provide skilled services in patients’ homes under the direction of a Physical Therapist. The PTA I educates patients and families, ensures safety, and maintains compliant documentation in line with state rules and Medicare guidelines.

Responsibilities include executing the plan of care, monitoring patient progress, performing prescribed PT procedures, and coordinating with the care team to support functional outcomes

Qualifications

• Ability to effectively communicate with patients, families, physicians and co-workers.
• Knowledgeable on therapy best practices and clinical judgment.
• Ability to identify situations and select appropriate PT-related solutions.

Responsibilities

• Provides skilled PTA services in the patient’s home as planned by the Physical Therapist.
• Observes and reports patient reactions, changes and progress.
• Performs prescribed PT procedures and diagnostic tests.
• Educates patients and families on therapy techniques, safety and equipment use.
• Completes documentation per agency and Medicare guidelines.
• Coordinates care with physicians, therapists and other staff.
