Job summary

Alternate Solutions Health Network is seeking a dedicated Physical Therapist Assistant to deliver skilled services in patients' homes under a Physical Therapist's plan of care. You will educate patients and families, document progress, and ensure safety and quality of care in the Dayton area.

The role requires an active PTA license in the state, at least one year of acute-care PTA experience, and a valid driver’s license.

Qualifications

• Licensed PTA in the state of employment.
• Minimum of one year of PTA experience in an acute setting.
• Home care experience is preferred.
• Ability to communicate clearly with patients, families, and team.

Responsibilities

• Provide skilled PTA services under the direction of a Physical Therapist.
• Educate patients and families on therapies and safety.
• Document patient progress and complete required charting on time.
• Travel to patient homes and participate in case conferences as needed.
• Assist with referrals and coordination with the care team.
