Qualifications

• Must hold a valid Physical Therapy license in the state(s) of practice and have a Master’s degree in Physical Therapy or an equivalent combination.
• Experience in outpatient or home-based PT settings is preferred.
• Ability to educate patients and caregivers on exercises, safety, and home programs.

Responsibilities

• Provide in-home outpatient rehabilitation to older adults, evaluating and treating in the patient’s home or independent living facility.
• Develop and implement individualized therapy plans to restore and improve function.
• Supervise PT assistants and other staff as required by state licensure.
• Document clinical notes and billing entries in a timely manner.
• Collaborate with the rehab team to refer to other disciplines when needed.
