Job summary

Powerback Rehabilitation is seeking a Physical Therapist for in-home outpatient services in New Hampshire. The role focuses on delivering high-quality PT in patients’ homes or in independent living facilities, with a strong emphasis on individualized plans and caregiver education.

The position offers a $5,000 sign-on bonus, a nationwide opportunity through our travel program, and visa sponsorship/relocation assistance.

Qualifications

Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
Master’s degree in Physical Therapy; or a Master’s in PT-related field plus a Bachelor’s in PT; or Bachelor’s in PT plus 5 years PT experience.

Responsibilities

Supervise Physical Therapy Assistants, temporary licensees, students and support staff as required by state licensure.
Adhere to department and patient treatment schedules; ensure timely clinical and billing documentation.
Attend patient care and staffing conferences and contribute to related meetings.
Refer patients to other disciplines to address comprehensive needs.
Collaborate with rehabilitation team to select appropriate therapy regimens.
Adapt plans of care based on patient needs and progress.
Promote a safe environment and eliminate hazards; order supplies and equipment as needed.
Document and maintain records of services and patient progress.
Design and implement home and community mobility assessments and interventions.
