The Miami VA Healthcare System Nursing Service is seeking an experienced Registered Nurse for Home Based Primary Care Outpatient. Ideal candidates contribute to quality care standards and VA academic hospital program growth.

Qualifications

English language proficiency required by VA policy.
Graduate from a nursing program approved by accrediting bodies.
License and/or certification requirements as RN per state board.
Foreign nursing education requires degree equivalency validation when applicable.

Responsibilities

Provide Home Based Primary Care (HBPC) to Veterans with oversight as needed.
Lead care coordination and case management for HBPC patients.
Plan and deliver technically competent nursing care at patients' homes.
Coordinate with patients, families, and interdisciplinary teams for safe transitions.
