Veterans Health Administration, part of the Department of Veterans Affairs, is seeking a Nurse Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse to provide and coordinate complex HBPC care for Veterans.

The role emphasizes leadership, experience, and creative approaches to management of complex HBPC care and includes a broad scope of clinical responsibilities within home-based settings.

Qualifications

• Current, unrestricted Registered Nurse license in any US state or territory.

Responsibilities

• Provides direct Veteran care and care coordination within the HBPC program.
• Assesses, plans, implements, and evaluates care using the nursing process.
• Administers medications and procedures per policies; collaborates with interdisciplinary team.
• Performs case management, triage, Veteran education, resource and medication coordination, and care planning.
• Delivers home-based nursing care to Veterans with complex needs to promote health and reduce unnecessary urgent care.
