Qualifications

- Graduate of a school of professional nursing approved and accredited by ACEN or CCNE; or current licensure with a state board.
- Foreign nursing education requires degree equivalency validation for licensure in the US.
Responsibilities
- Provide direct Veteran care via home visits and care coordination within the HBPC program.
- Perform case management, assessment, triage, Veteran education, resource coordination, medications and treatment coordination; care planning.
- Deliver technically competent home-based care, emphasizing health promotion and prevention to improve outcomes and reduce emergency visits.
- Collaborate on policy/guideline development and provide feedback for quality improvement.
- Support colleagues with HBPC knowledge sharing and maintain professional development.
