Job summary

Veterans Health Administration, part of the Department of Veterans Affairs, seeks an experienced Nurse Practitioner for Home Based Primary Care and Geriatrics/Extended Care to deliver comprehensive patient care in Sebring, FL.

The NP will independently manage a panel of Veterans, conduct admission assessments, and collaborate with the HBPC team to promote health, disease prevention, and evidence-based practice in diverse living arrangements.

Qualifications

• Licensed Independent Provider with authority to practice in the appropriate US state.
• Ability to work autonomously and in coordination with health care professionals.
• Strong communication and teamwork skills.

Responsibilities

• Provide direct patient care in Home Based Primary Care and Geriatrics/Extended Care settings.
• Perform admission H&P, progress notes, discharge notes, and daily patient management.
• Lead and participate in Interdisciplinary Team meetings.
• Coordinate care with HBPC team and community-based services.
• Respond to HBPC emergencies per directives and participate in care planning.
