The Nurse Practitioner - Home Base Primary Care position is with the Geriatrics and Extended Care Service of the VA White River Junction Healthcare System, located in Littleton, NH. This is a full-time, 40-hour-per-week role with competitive federal compensation and benefits.

Responsibilities

• Independently manages a panel of patients with acute and chronic health conditions with emphasis on health promotion and disease prevention
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and collaborates with the interdisciplinary team
• Orders and interprets labs, tests, and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Educates patients and families about plan of care and self-management
• Monitors and adjusts the plan of care to achieve optimal outcomes
