Qualifications

• Education: post-master's certificate or master's or doctoral degree as a Nurse Practitioner (including pending completions).
• License: active, current, full APN license to practice as an NP in the U.S.
• Board Certification: FNP/ANP/AGPCNP/GNP from ACEN/CCNE or equivalent.

Responsibilities

• Provide advanced clinical practice in Home Based Primary Care within Geriatrics and Extended Care Service.
• Deliver a full range of health care services emphasizing quality and timeliness.
• Lead and participate in interdisciplinary teams to improve veteran/patient care.
