Job summary

The Advanced Practice Nurse (APN) III Certified NP in HBPC delivers comprehensive health care to Veterans in geriatrics and extended care, focusing on quality and timeliness in line with VA mission. The NP provides advanced clinical practice, consultation, and care management.

Duties include home visits, independent patient management, order labs, interpret results, and collaborate with the HBPC team to develop and adjust treatment plans that improve health outcomes.

Qualifications

• Active NP license in any US state or territory.
• Ability to provide comprehensive care in geriatrics.
• Proven experience in home-based primary care is preferred.

Responsibilities

• Independently manages a panel of patients with acute and chronic conditions using evidence-based practice.
• Visits patients in their homes in the community.
• Drives a vehicle to patient visits in the community and adheres to vehicle policies.
• Develops and implements individualized treatment plans to achieve optimal outcomes.
• Collaborates with HBPC interdisciplinary team to solve complex patient problems.
