Job summary

Anacalifornia seeks an experienced Advanced Practice Nurse (APN) III Certified Nurse Practitioner (NP) to provide Home Based Primary Care (HBPC) within the Geriatrics and Extended Care Service for Veterans Affairs. The NP delivers advanced clinical practice, consultation, and leadership in collaboration with the HBPC team.

The role includes home visits, patient-centered care, and coordination with VA staff to ensure timely, high-quality health services and comprehensive clinical documentation.

Qualifications

• Must hold 1 full and unrestricted license from any US State or territory.

Responsibilities

• Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
• Visits patients in their homes in the community.
• Orders and interprets labs, tests, and diagnostic studies.
• Develops and implements an individualized treatment plan to achieve optimal patient health outcomes.
• Provides clinical coverage when other NPs are on leave.
