Job summary

HarmonyCares is seeking a Nurse Practitioner to provide in-home primary care for high-acuity patients across select markets. This role emphasizes patient-centered care, thoughtful assessment, and time to address each patient’s needs rather than volume.

You will be licensed as an NP in the relevant state with DEA eligibility, CPR/BLS, and geriatric experience preferred. A company car or travel support may be provided in select markets.

Qualifications

• Licensed as a NP in the state where HarmonyCares Medical Group office located.
• Federal DEA license/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Valid and unencumbered driver's license with good driving record

Responsibilities

• Provide home-based primary care to high acuity patients in the patient’s home.
• Take the time needed to address the patient’s needs, not volume.
• Collaborate with the care team to coordinate home health and geriatrics services.
