Nurse Practitioner or Physician Assistant – Home-Based Primary Care | Newark area, NJ

HarmonyCares is a leading national provider of value-based, in-home primary care for patients with complex healthcare needs. We are seeking a Nurse Practitioner to serve patients in Newark, NJ and limited surrounding areas.

This role is ideal for an NP who wants to focus on patient care, not volume. Our providers typically see 6–10 patients per day, with approximately 30–60 minutes per visit, allowing time to manage complex and chronic conditions and build meaningful relationships with patients and families.

What You'll Do

Provide comprehensive primary care to medically complex adult patients in their homes, including:

• Assessing, diagnosing, and treating acute and chronic conditions
• Managing patients with multiple comorbidities

• Prescribing and managing medications
• Educating patients and caregivers
• Collaborating with the interdisciplinary care team

Qualifications

• Active New Jersey Nurse Practitioner or Physician Assistant license
• Board certification
• Federal DEA license or eligibility
• Active CPR/BLS certification
• Valid driver's license and good driving record
• Primary care experience with adult patients
• Ability to travel locally throughout the assigned territory

Experience in geriatrics, home-based care, home health, hospice, palliative care, or complex-care populations is preferred.
