Job summary

HarmonyCares seeks a Nurse Practitioner to deliver in-home primary care to high-acuity patients. This role emphasizes patient-centered, relationship-based care rather than volume, with time to address complex needs in patients' homes.

The ideal candidate will be licensed as an NP or PA in the relevant state, hold a DEA license, and bring geriatric experience, CPR/BLS certification, and board certification where applicable. A valid driver’s license is required.

Qualifications

• Licensed as a NP or PA in the state where HarmonyCares Medical Group operates.
• DEA license/eligibility required.
• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board certification preferred or required for practice.

Responsibilities

• Provide home-based primary care to high-acuity patients.
• Practice medicine with a focus on patient care, not volume.
• Take time to address each patient’s needs and coordinate care.
