Job summary

Medix is seeking a compassionate Nurse Practitioner to deliver home-based primary care to patients in Virginia. You will manage a patient panel, perform home visits across private residences and assisted living facilities, and collaborate with a clinical leadership team to optimize outcomes and reduce hospital visits.

The role requires Virginia NP license, DEA license, and a minimum of 3 years of primary care experience.

Qualifications

Active NP license in Virginia.
Active DEA license or eligibility to obtain prior to start.
Minimum of 3 years of clinical experience in primary care.
Strong background in Chronic Disease Management.
Valid driver's license with an acceptable driving record.
Comfort conducting home visits across diverse environments.

Responsibilities

Conduct home-based primary care visits in residences and assisted living facilities (4–8 patients per day).
Perform follow-ups (30 min), post-hospitalization evaluations (45 min), and new patient assessments (60 min).
Manage chronic conditions to improve outcomes and reduce hospital visits.
Work independently or with a Medical Assistant, with efficient travel schedules.
Utilize Athena EMR for documentation and patient management.
Participate in weekly team sessions and panel reviews.
Maintain safety and infection control standards in the field.
