Qualifications

• Active state NP license.
• Current NP National Board Certification.
• Active state DEA licensure.
• MSN degree.
• Five+ years nursing experience; NP experience preferred.
• Strong interpersonal communication skills.
• Excellent time management and accountability.
• Proficiency with medical instruments and EMR systems.
• HIPAA and OSHA knowledge.
• Proficient with Microsoft Office applications.
• Valid driver’s license with auto insurance.
• Ability to perform moderate physical activity.

Responsibilities

• Performs primary care for seniors in home settings.
• Conducts clinical assessments and creates care plans.
