Job Title: Nurse Practitioner - Home-Based Primary Care
Job Summary
We are looking for a compassionate, dependable, and highly skilled Nurse Practitioner to join a growing team. In this role, you will deliver high-quality primary care services directly to patients within their homes and assisted living facilities. Supported by a highly collaborative clinical leadership team, you will manage a patient panel with a focus on comprehensive care, chronic disease management, and minimizing unnecessary hospital visits.
Responsibilities
- Conduct home-based primary care visits in private residences and assisted living facilities, seeing approximately 4 to 8 patients per day.
- Perform varied appointment types including follow-up visits (30 minutes), post-hospitalization evaluations (45 minutes), and comprehensive new patient assessments (60 minutes).
- Manage chronic conditions and deliver personalized primary care to help improve patient outcomes and reduce acute hospital visits.
- Practice independently or alongside a Medical Assistant in the field, utilizing efficient daily travel schedules optimized by local scheduling teams.
- Utilize Athena EMR for documentation, patient management, and communication.
- Participate in weekly team collaboration sessions, including staff meetings, high-risk patient huddles, and panel management reviews.
- Maintain flexibility to adapt to field conditions while adhering to safety protocols and infection control standards.
Schedule
- Monday through Friday, 8:30 a.m. - 5:00 p.m.
- Full-time schedule with structured patient contact and travel built directly into the workday.
- 4-day workweek flexibility options available.
Required Qualifications
- Active Nurse Practitioner (NP) license in the state of Virginia.
- Active DEA license (or eligibility to obtain prior to start).
- Minimum of 3 years of clinical experience in primary care.
- Strong background in Chronic Disease Management.
- Valid driver's license with an acceptable driving record.
- Comfort and willingness to conduct home visits across diverse residential environments.
Preferred Qualifications
- Prior experience working with geriatric populations, home health, or hospice care.
- Hands-on experience with Athena EMR software.
