Rehab Without Walls® Neuro Rehabilitation in Ann Arbor, MI is looking for an experienced Physical Therapist to join our Home and Community Program. The role emphasizes providing functional rehabilitation in patients' homes and community settings, offering flexibility and support.

Formación

• Degree in Physical Therapy from an accredited program.
• Active Physical Therapist license in practicing state.
• Certification in Basic Life Support or CPR as per state regulations.
• At least one year of physical therapy experience preferred.

Responsabilidades

• Perform patient evaluations and develop individualized treatment plans.
• Regularly reassess treatment plan effectiveness.
• Conduct discharge evaluations, recommending safety modifications.
• Communicate patient needs to the interdisciplinary team.
• Collaborate across disciplines to improve patient outcomes.
