Job summary

HarmonyCares is seeking a Nurse Practitioner to deliver in-home primary care to high-acuity patients within our home-based program. You will focus on patient-centered care, not volume, and work Monday through Friday with dedicated support staff.

This role requires NP licensure in the state, DEA license eligibility, CPR/BLS, geriatric experience, and board certification. We offer competitive salary, comprehensive benefits, CME stipend, and tail malpractice coverage.

Qualifications

• Licensed NP in the state where HarmonyCares Medical Group office is located.
• Federal DEA license or eligibility required.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification required.
• Board Certification in nursing.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide in-home primary care to high-acuity patients.
• Deliver patient-centered care with time to address patient needs.
• Collaborate with care teams to support home-based services.
