Rehab Without Walls Neuro Rehabilitation in Irvine, CA is seeking a licensed Physical Therapist for home and community-based neuro rehabilitation. This PRN role focuses on evaluating patients, planning individualized treatments, and driving independence in daily life.

Qualifications

• Minimum of bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license by the practicing state.
• CPR/BLS certification in accordance with state regulations.
• A minimum of one year's work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques for complex neurological injury.
• Ability to communicate effectively in verbal and written form.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness, attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Coordinate with other disciplines to ensure collaboration and better patient outcomes.
