Job summary

Rehab Without Walls Neuro Rehabilitation in Thousand Oaks is seeking a licensed Physical Therapist to provide neuro rehabilitation services in patients' homes and community settings. The role emphasizes individualized, one-on-one care and ongoing assessment of progress within a multidisciplinary team.

This PRN position requires a Bachelor's in Physical Therapy, active state licensure, and BLS/CPR certification.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• BLS or CPR certification as required by state regulations.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess treatment effectiveness and attend meetings.
• Perform discharge evaluations and home safety recommendations.
• Communicate patient needs and progress to the care team and family.
• Collaborate with other disciplines to improve patient outcomes.
