Job summary

Rehab Without Walls Neuro Rehabilitation in California is seeking an experienced Physical Therapist to extend rehabilitation beyond the clinic and into clients' homes, schools, workplaces, and communities.

You will evaluate and plan treatment, reassess progress, coordinate with families and care teams, and support clients to perform daily activities safely and independently in real-world environments.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapy license in the practicing state.
• Current Basic Life Support/CPR certification as required by state regulations.
• Minimum of one year's work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques for neurological injury.
• Excellent communication skills, both verbal and written.
• Ability to lift and move patients as needed per safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment progress and participate in staffing meetings and family conferences.
• Perform discharge evaluations and provide home and community safety recommendations, plus referrals and education resources.
• Communicate patient needs and progress to the treatment team, physician, patient and family members.
• Collaborate with other disciplines to coordinate care and enhance patient outcomes.
