Job summary

Granite VNA, New Hampshire’s largest home health and hospice provider, seeks a Medical Social Worker (MSW) for our home-care teams across the Lakes Region. The role offers autonomy, Monday–Friday scheduling, and a focus on compassionate care for patients at home.

You will assess needs, provide counseling, coordinate with the care team, and help families navigate illness, end-of-life planning, and discharge preparation. Competitive pay and benefits accompany this community-based position.

Qualifications

• MSW required or BA in social work with healthcare experience.
• Experience in home health or hospice is preferred.
• Strong counseling and education delivery to patients and families.

Responsibilities

• Assess patient and family needs and develop individualized plans of care.
• Provide counseling, behavioral health interventions, education, and community resource support.
• Coordinate care with physicians and interdisciplinary team members.
• Document patient progress, interventions, and outcomes per agency standards.
• Conduct hospice information visits for home care patients when requested.
• Support patient and caregiver education and discharge planning.
• Mentor and support new MSW staff and students.
