Job summary

Granite VNA, New Hampshire’s largest home health and hospice provider, seeks a Medical Social Worker (MSW) to support patients and families in home settings. The role offers autonomy, Monday–Friday, 8am–4pm, with pay of $35.00–$45.00 per hour plus mileage reimbursement and a comprehensive benefits package.

Reporting to the Director, you will coordinate care with physicians and the interdisciplinary team, provide counseling, and document outcomes.

Qualifications

• Master's degree in social work (CSWE-accredited) or bachelor's in social work with at least 1 year in a healthcare setting.

Responsibilities

• Assess patient and family needs and develop individualized plans of care.
• Provide counseling, behavioral health interventions, education, and community resource support.
• Coordinate care with physicians and interdisciplinary team members.
• Document patient progress, interventions, and outcomes per agency standards.
• Conduct hospice information visits for home care patients as requested.
• Support patient and caregiver education and discharge planning.
• Mentor and support new MSW staff and students.
