The role offers autonomy, Monday–Friday, 8am–4pm, with pay of $35.00–$45.00 per hour plus mileage reimbursement and a comprehensive benefits package.

Reporting to the Director, you will coordinate care with physicians and the interdisciplinary team, provide counseling, and document outcomes.

Qualifikationen

• Master's degree in social work (CSWE-accredited) or bachelor's in social work with at least 1 year in a healthcare setting.

Position Overview

Reporting to the Director, the MSW provides age-appropriate counseling, education, and support to patients and families in the home setting using standard social work practices. This role partners closely with patients, caregivers, physicians, and the interdisciplinary care team to develop and coordinate individualized plans of care.
