Job summary

Granite VNA in Concord, NH is seeking a dedicated Medical Social Worker (MSW) to join our Hospice House team. In this on-site role, you will deliver psychosocial care, support patients and families, and collaborate with an interdisciplinary team to ensure dignified end-of-life care.

Responsibilities include comprehensive psychosocial assessments, care planning, education about hospice services and community resources, and timely documentation in compliance with Medicare guidelines.

Qualifications

• Masters of social work degree (MSW) from a school of social work accredited by the Council on Social Work Education
• One year of Social Work experience in a healthcare setting
• Interest in working with terminally ill patients and families

Responsibilities

• Provide compassionate counseling and emotional support to patients, families, and loved ones throughout the hospice journey and during bereavement.
• Complete comprehensive psychosocial assessments and develop individualized care plans that address each patient's unique needs.
• Educate patients and families about hospice services, grief, loss, advance care planning, and available community resources.
• Collaborate with nurses, physicians, chaplains, and other interdisciplinary team members to provide holistic, patient-centered care.
• Assist families with care planning, living arrangement transitions, discharge planning, and connections to financial or community resources.
• Facilitate meaningful communication between patients, families, and the care team to ensure goals of care are understood and honored.
• Maintain accurate, timely documentation in accordance with agency policies and Medicare requirements.
• Participate in interdisciplinary team meetings, quality improvement initiatives, and educational opportunities.
• Support and mentor new social work staff and students as opportunities arise.
