Job summary

Granite VNA, Inc. in New Hampshire’s Lakes Region is seeking a Medical Social Worker (MSW) for a Monday–Friday, 8am–4pm schedule in the Lakes Region area. This community-based role provides in-home counseling and planning with competitive pay and mileage reimbursement.

You will support patients and families through emotional, social, and practical challenges of illness and end-of-life care, collaborating with physicians and an interdisciplinary care team.

Qualifications

• Master's degree in social work (CSWE) OR bachelor's degree in social work with at least one year in healthcare.

Responsibilities

• Assess patient and family needs and develop individualized plans of care.
• Provide counseling, behavioral health interventions, education, and community resource support.
• Coordinate care with physicians and interdisciplinary team members.
• Document patient progress, interventions, and outcomes in accordance with agency standards.
• Conduct hospice information visits for home care patients when requested.
• Support patient and caregiver education and discharge planning.
• Mentor and support new MSW staff and students.

Position Overview

Reporting to the Director, the MSW provides age-appropriate counseling, education, and support to patients and families in the home setting using standard social work practices. This role partners closely with patients, caregivers, physicians, and the interdisciplinary care team to develop and coordinate individualized plans of care.

Requirements

1. Master's degree in social work from an accredited program (Council on Social Work Education) OR bachelor's degree in social work with at least one year in healthcare care setting.
