Job summary

Lakes Region VNA and Hospice seeks a compassionate Social Worker to support Hospice and Home Care patients and families. Responsibilities include psychosocial assessments, care planning, and emotional support to navigate healthcare challenges.

The ideal candidate holds an MSW with at least one year in healthcare, NH social work licensure, and strong communication skills. Travel within the service area is expected; competitive benefits apply.

Qualifications

• Master’s degree in social work (MSW) from an accredited program.
• Minimum of one year of healthcare experience, preferably hospice or home care.
• Current and valid Social Worker license in New Hampshire.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Develop and implement individualized care plans with the interdisciplinary team.
• Provide counseling and support for illness, loss, and end-of-life planning.
• Coordinate access to community resources and referrals.
• Respond to crises and ensure patient safety and housing needs are met.
• Document all interactions and care plans timely in the medical record.
• Collaborate with nurses, physicians and other professionals to ensure comprehensive care.
• Educate patients and families on rights and available services; advocate within the system.
• Support hospice patients and families, including bereavement assistance.
