Job summary

HopeHealth, Inc. is seeking an experienced Hospice Social Worker (LICSW) in Providence, RI. This full-time role focuses on planning, delivering and evaluating social work services for patients in home settings within Woonsocket and surrounding areas.

The Clinical Social Worker will collaborate with an interdisciplinary team, perform psychosocial assessments, document in EMR, and participate in on-call coverage. Licensure and MSW are required to qualify for this position.

Qualifications

• Master of Social Work degree required.
• Licensed LICSW in the state you are applying for.

Responsibilities

• Completes psychosocial assessments.
• Basis social work services on patient/family needs and acceptance.
• Collaborates with Interdisciplinary Team on social/emotional factors in plan of care.
• Provides complete and accurate EMR documentation.
• Travel to patient homes within the service area.
• Participates in on-call rotation for weekends and holidays.
