Job summary

Providence at Home with Compassus is seeking a compassionate Home Health Social Worker to assess psychosocial, financial, environmental, and community needs, and to support patients and families toward optimal functioning and quality of life. You will provide crisis intervention, grief support, and counseling on end‑of‑life issues while coordinating with families, care teams, and referral sources.

You will contribute to care planning, ensure regulatory compliance, and assist with discharge

Qualifications

• Master's degree in Social Work (MSW) is required.
• 2 years of supervised, post‑Master’s experience in a psychiatric or other health care setting is preferred.

Responsibilities

• Conducts psychosocial assessments and documents findings in the Plan of Care and records.
• Delivers direct and indirect social services across home, long‑term care facilities, and hospitals.
• Provides crisis intervention and supportive counseling for terminal illness and end‑of‑life issues.
• Complies with federal/state regulations and organizational policies in documentation and duties.
• Participates in interdisciplinary team meetings to support care planning and coordination.
• Consults with colleagues, leadership, patients, and families to support care goals.
• Facilitates level‑of‑care transitions and discharge planning.
• Responds to social services referrals from the hospice team.
• Fosters a collaborative, team‑based approach within professional competencies.
