Pathways Healthcare is a leading home health and hospice organization focused on patient-centered care. We seek a Social Worker (MSW) for Worcester with LICSW preferred, two+ years hospice experience, CPR, and a valid license.

You will work Mon-Fri 8:30-5:00, travel to patient homes, and collaborate with an interdisciplinary team to deliver compassionate end-of-life support. The role emphasizes maintaining EMR records, coordinating care, and guidance for families, with strong emphasis on dignity

Qualifications

• Masters of Social Work from an accredited program.
• LICSW license in state of practice preferred; LCSW considered.

Responsibilities

• Provide psychosocial assessments and emotional support to hospice patients and families.
• Counsel and guide patients, families, and caregivers.
• Coordinate with interdisciplinary hospice team to support care plans.
• Participate in IDG meetings and daily briefings for coordinated care.
• Connect patients with community resources and support services.
• Maintain accurate documentation in the EMR system.
• Communicate patient status updates to the clinical team.
• Support families through grief and end-of-life planning.
• Follow confidentiality, safety, and infection control protocols.
• Participate in case reviews, training, and on-call duties.
