Job summary

Providence at Home with Compassus is seeking a compassionate Hospice Social Worker to support patients and families across homes, facilities, and hospitals. You will assess psychosocial needs, provide crisis intervention, and coordinate with care teams while contributing to discharge planning.

Requires a Master’s in Social Work and WA licensure; 1 year in health care is preferred. This role emphasizes holistic, patient-centered care and collaboration across settings.

Qualifications

• Master's degree from an accredited School of Social Work.
• Preferred: 1 year post-Master’s experience in a health care setting.
• Washington licensure paths (Vendor Managed) including Advanced Social Worker or Clinical Independent Social Worker licenses.

Responsibilities

• Conducts psychosocial, financial, environmental, and community needs assessments and documents in care plans.
• Provides crisis intervention and supportive counseling to patients and families coping with terminal illness.
• Delivers direct and indirect social services across homes, long-term care facilities, and hospitals.
• Participates in interdisciplinary team meetings to support care planning and coordination.
• Facilitates level-of-care transitions and discharge planning; supports regulatory compliance and documentation.
