Job summary

Providence at Home with Compassus, a leader in home-based hospice and palliative care, seeks a compassionate Hospice Social Worker to support patients and families across homes, long-term care facilities, and hospitals. You’ll use clinical judgment to assess psychosocial, financial, environmental, and community needs.

Qualifications

• Master's degree from an accredited School of Social Work.
• Preferred – 1 year of post-Master’s experience in a health care setting.

Responsibilities

• Conducts clinical assessments to identify patients' psychosocial, financial, environmental, and community needs.
• Delivers social services across homes, long-term care facilities, and hospitals.
• Provides crisis intervention and supportive counseling for terminal illness.
• Completes documentation in compliance with federal/state regulations.
• Participates in interdisciplinary team meetings and care planning.
• Consults with colleagues, leadership, patients, families, and external partners.
• Facilitates level-of-care transitions and discharge planning.
