Resource 1 Homecare Staffing in Clarinda, IA is seeking a Hospice Registered Nurse (RN) Case Manager to provide compassionate, patient-centered hospice care in home and community settings. The role is 40 hours per week and requires nursing licensure and hospice/acute care experience.

Responsibilities include comprehensive patient assessments, care planning, coordination with the interdisciplinary team, and on-call duties.

Qualifications

• Graduation from an accredited school of nursing.
• 1–2 years of recent acute care in an institutional setting.
• 1–2 years of recent home care experience preferred.
• Valid driver's license and reliable automobile.

Responsabilités

• Completes initial and ongoing patient assessments to determine hospice needs.
• Develops and revises the plan of care with nursing interventions.
• Regularly re-evaluates patient and family needs and coordinates care.
• Initiates and revises the plan of care as patient status changes.
• Provides nursing care and mediates health education in the home.
• Coordinates with the interdisciplinary team and participates in on-call duties.
