Job summary

HarmonyCares is seeking a Nurse Practitioner to provide comprehensive, patient-centered primary care in home settings for high-acuity patients. The role emphasizes quality patient interaction over volume and leverages a team-based, physician-driven model.

The NP should be licensed in the state of practice, hold DEA eligibility, and maintain CPR/BLS certification, with geriatric experience preferred. A valid driver’s license and good driving record are required.

Qualifications

• Licensed as a NP in the state where HarmonyCares Medical Group office is located.
• Federal DEA license/eligibility.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide primary care to high acuity patients in their home.
• Take the time needed to truly address the patient’s needs, not volume.
• Practice medicine with focus on patient care in home health, geriatrics, hospice, and primary care.
• Assistant to support appointments in select markets.
