HarmonyCares seeks a Nurse Practitioner to provide high-quality, patient-centered in-home primary care for high-acuity patients. This role emphasizes geriatrics and hospice care, delivering compassionate medical services directly in patients' homes.

The clinician will balance comprehensive assessments with thoughtful patient interactions, ensuring thorough care plans and coordination with the broader care team. A valid NP license, DEA eligibility, and CPR/BLS certification are required.

Qualifications

• Licensed as an NP in the state(s) where HarmonyCares Medical Group operates.
• DEA license/eligibility required.
• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board certification as NP.
• Valid driver's license with good driving record.

Responsibilities

• Deliver patient-centered home health care for high-acuity patients in their homes (geriatrics, hospice, primary care).
• Take time to address the patient’s needs beyond volume and throughput.
• Collaborate with care teams to coordinate services and ensure quality outcomes.
