Job summary

Pinnacle Family Services, part of Health Connect America, is seeking a Family Centered Treatment (FCT) Specialist to serve children and families in home and community settings across North Carolina. You will guide families through the structured FCT model, coordinating with a Treatment Team and developing customized Service Plans.

This role emphasizes hands-on, relationship-driven care with flexible scheduling and on-call availability.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• One of the following: Master’s in human services + 1 year experience; Bachelor’s in human services + 2 years; Bachelor’s in non-human services + 4 years.
• Current NC license or board‑issued certification in a human services profession.
• Full‑time, supervised experience serving children/adolescents with a primary mental health diagnosis.
• Valid driver’s license and access to insured transport.

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings.
• Guide children and families through the four phases of the FCT model.
• Serve as central point of coordination for each family's Treatment Team.
• Develop and refine individualized Service Plans with families.
• Connect families to community resources and aftercare supports.
• Maintain accurate documentation in the Electronic Health Record.
• Participate in Quality Assurance activities to improve program strength.

Education

Master's in human services field + 1 year experience

Bachelor's in human services field + 2 years experience

Bachelor's in non-human services field + 4 years experience

Current NC license (including provisional) or board‑issued certification in a human services profession

Experience must be full‑time and supervised, serving children and adolescents with a primary mental health diagnosis

Must have a valid driver’s license and access to a fully insured, private transportation
