Job summary

Pinnacle Family Services, part of Health Connect America, is hiring a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in homes and communities across North Carolina. You will work with children, adolescents, and families, guiding them through a structured treatment model designed for lasting change.

The role emphasizes hands-on, relationship-driven care, with a focus on coordination with a Treatment Team and ongoing service planning.

Qualifications

• Must meet NC QP requirements per 10A NCAC 27G .0104 and hold related licensure.
• Relevant Master's or Bachelor's degrees with supervised experience in youth mental health.
• Licensed or certified in North Carolina and able to drive to homes and communities.

Responsibilities

• Deliver Family Centered Treatment services in home and community settings.
• Guide families through the four phases of the FCT model.
• Coordinate with medical, behavioral health, school and community partners.
• Develop and refine individualized Service Plans with families.
• Connect families to community resources and aftercare supports.
• Maintain timely documentation in the Electronic Health Record.
• Participate in Quality Assurance activities to improve programs.
