Job summary

Pinnacle Family Services in North Carolina is seeking a Family Centered Treatment (FCT) Specialist to serve children and families in home and community settings. You will deliver evidence-based services in partnership with families, guiding them through the FCT model and coordinating with the Treatment Team.

The role requires NC QP qualifications, relevant degrees, a valid driver’s license, and full-time supervised experience; a flexible schedule and on-call crisis support are part of the role,

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Master's in human services field + 1 year experience.
• Bachelor's in human services field + 2 years experience.
• Bachelor's in non-human services field + 4 years experience.
• Current NC license or board-issued certification in a human services profession.
• Full-time supervised experience serving children/adolescents with a primary mental health diagnosis.
• Valid driver’s license and access to private transportation.

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings.
• Guide children and families through the four phases of the FCT model.
• Serve as central coordinator for each family’s Treatment Team with partners across medical, behavioral health, school and community services.
• Develop, implement, and refine individualized Service Plans with families.
• Connect families to community resources, referrals, and aftercare supports.
• Maintain timely documentation in the Electronic Health Record reflecting quality care.
• Participate in Quality Assurance activities to strengthen programs.
