Job summary

Pinnacle Family Services is seeking a Family Centered Treatment Specialist to deliver evidence-based services in homes and communities. You’ll guide families through structured treatment phases and coordinate with medical, behavioral health, school, and community partners to ensure comprehensive care.

FCT Certification is required within one year of hire. A NC Qualified Professional credential, supervision, and a valid driver’s license are essential for this role.

Qualifications

• Meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Master's in human services field + 1 year experience.
• Bachelor's in human services field + 2 years experience.
• Bachelor's in non-human services field + 4 years experience.
• Current NC license or board-issued certification in a human services profession.
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis.
• Must have a valid driver’s license and access to transportation.

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings.
• Guide children and families through the four phases of the FCT model, helping them set goals and build skills.
• Serve as a central point of coordination for each family's Treatment Team with partners for seamless care.
• Develop, implement, and refine individualized Service Plans with families.
• Connect families to community resources, referrals, and aftercare supports.
• Maintain timely documentation in the Electronic Health Record and participate in QA activities.
