Pinnacle Family Services in North Carolina seeks an experienced Family Centered Treatment (FCT) Specialist to deliver evidence-based services in homes and communities. You will work with children and families to guide them through the FCT model and coordinate care with partners to ensure whole-person healing.

The role is community-based with on-call crisis support and certification within one year. A NCQP, appropriate degree, driving eligibility, and a valid driver's license are required.

Formación

• NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104 and one of the following
• Master's in human services field + 1 year experience
• Bachelor's in human services field + 2 years experience
• Bachelor's in non-human services field + 4 years experience
• Current NC license (including provisional) or board-issued certification in a human services profession
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis
• Must have a valid driver's license and access to a fully insured, private form of transportation

Responsabilidades

• Deliver Family Centered Treatment services primarily in home and community settings
• Guide children and families through the four phases of the FCT model
• Serve as a central point of coordination for each family's Treatment Team
• Develop, implement, and refine individualized Service Plans with families
• Connect families to community resources, referrals, and aftercare supports
• Maintain timely documentation in the Electronic Health Record
• Participate in Quality Assurance activities to strengthen programs
